This is the initial, in person evaluation to assess you and make a plan of care.

GET READY FOR YOUR APPOINTMENT

Collect any documents or previous test results to bring to your first appointment. Take a moment to fill out the New Patient Paperwork. There will also be a link in your appointment email.

SAFETY NOTES

Due to the close nature of manual therapy, face masks are required to be worn upon entering appointment.
